Avascular Necrosis Treatment with Hyperbaric Oxygen Therapy

Avascular necrosis occurs when bone tissue dies from a loss of blood supply, most often in the hip. HBOT may support new blood vessel growth in the affected bone and help avoid or delay joint replacement surgery.

Understanding Avascular Necrosis

Avascular necrosis (AVN), also called osteonecrosis, occurs when blood supply to a bone is disrupted, causing the bone tissue to die. The hip is the most commonly affected joint, but AVN can also occur in the knee, shoulder, and other bones.

Risk factors include long-term corticosteroid use, excessive alcohol consumption, traumatic injuries, certain autoimmune conditions, and some blood disorders. As the bone weakens, it can eventually collapse, leading to severe joint pain and loss of function. Without treatment, many patients with AVN ultimately require joint replacement surgery.

How Hyperbaric Oxygen Therapy Helps

HBOT increases oxygen delivery to the affected bone, which may help stimulate the growth of new blood vessels (angiogenesis) in the area of dying bone. The UHMS recognizes AVN as an indication for HBOT based on evidence suggesting that early treatment may slow or halt the progression of bone death.

By improving the oxygen environment within the bone, HBOT supports the body's natural bone repair processes. In some patients, particularly those in earlier stages of AVN, this may help preserve the joint and avoid or delay the need for joint replacement.

What to Expect During Treatment

Treatment typically involves 20 to 40 sessions, each lasting approximately 90 minutes. Your orthopedic surgeon and hyperbaric physician work together to determine the best treatment plan based on the stage and location of your AVN.

This indication is recognized by the Undersea and Hyperbaric Medical Society (UHMS) and supported by peer-reviewed clinical evidence. It is not currently covered by Medicare under NCD 20.29.
